Please allow us 1-2 business days after we receive your request to follow up with available appointment times and answer any questions you may have. Also, please note that our schedule is very full, and it can take several weeks to get your first appointment.

TO SCHEDULE AN APPOINTMENT WITH US
STEP 1: COMPLETE THE INTAKE FORM
To schedule an appointment with us, please fill out the Intake Form.
Due to the specialized nature of our practice, we are usually booked 6 to 8 weeks in advance.
Please return the form to: firstname.lastname@example.org.
Staff will contact you within two business days by phone or email to secure a date in our calendar at one of our four locations.
Please make every effort to download the form and send it to us, but we do understand that technology doesn't always cooperate. If you have trouble with the form, please call 630-231-1544 to leave a message, and we will call you back within two business days.
STEP 2: AFTER YOU SEND IN YOUR INTAKE FORM
- Once we receive your intake form we, will contact you by phone or email in order to schedule your initial appointment.
We will contact you within two business days to schedule your consultation.
A non-refundable scheduling fee of $100 is required to schedule a new patient consultation. We will ask for this fee to be paid (by credit card) once your consultation is scheduled. The scheduling fee is fully refunded at the end of your initial appointment.
You will forfeit your scheduling fee if you reschedule your appointment with less than 72-hours notice or do not show to your appointment.
You may reschedule your appointment with 72-hours notice once before losing your scheduling fee.
STEP 3: ONCE YOUR APPOINTMENT HAS BEEN SCHEDULED
The following forms are utilized by Chicagoland Veterinary Behavior Consultants (CVBC) in order for us to collect preliminary information regarding your pet's behavior problem. We will be discussing the full details of your pet's behavior at the time of the consultation.
Once you have completed these forms, save them on your computer, then attach them to an email which can be sent to email@example.com. They may also be faxed to 630.554.0214.
