ONCE YOUR APPOINTMENT IS SCHEDULED THERE ARE IMPORTANT PREPARATORY STEPS to ensure we have the most accurate understanding of your pet’s history and health. The more detailed information we receive, the better we can assess your pet’s behavior and develop an effective treatment plan.

1. Complete the Behavior History Questionnaire

2. Provide Your Pet’s Medical Records

Behavioral concerns are often influenced by underlying medical conditions, so we must evaluate both your pet’s physical and mental health. Over 50% of our patients have either pre-diagnosed or unrecognized medical conditions affecting their pet's behavior

  • Authorize the release of medical records from your primary care veterinarian, or any emergency, or specialist veterinarian who has treated your pet.

    • At a minimum, we require records from the past 12 months.

    • If your pet has ever been evaluated for behavior problems, prescribed behavioral medication, or treated for a behavioral issue—prior to the past 12 months—please request release of those records as well.

    • If your pet has a significant or chronic medical condition, we will also need those records, regardless of when the diagnosis was made.

  • Please provide us with the names and email addresses of the veterinarians who have treated your pet in case we have additional questions.

3. Submit Recent Bloodwork Results

To help rule out medical conditions that may impact behavior, in addition to prior medical records, we require recent bloodwork to be completed through your primary care veterinarian before your appointment.

  • Required tests:

    • Complete Blood Count (CBC)

    • Biochemistry Profile

    • Total T4 (thyroid screening test)

  • If your pet has any issues with urination or excessive drinking, we also recommend a urinalysis.

  • If your pet has had these tests done within the last 3–4 months, there is no need to repeat them—please send us the existing results.

  • If it is too distressing or dangerous to obtain a blood sample from your pet, these tests can be postponed until after the consultation.

Ensuring we receive this information before your consultation allows us to dedicate more time to discussing your pet’s behavior and treatment plan during your appointment.
